The Ontario Community Improvement Department administers various fees and fines for multiple programs throughout the City, as well as for violations of the Ontario Municipal Code (OMC).
If you received a Community Improvement fee/fine and believe it was placed in error, or if there are exigent circumstances which should lead to the fee or fine being dismissed, you can request an informal fee review by completing this form.
You may contest an administrative citation by submitting this form within 15 calendar days from the date the citation was issued if the violation did not occur or if you are not the responsible party. There is no charge for this review. If an initial review is not requested in a timely manner, the administrative citation stands and cannot be contested further.
If you received a parking citation from the Community Improvement Department and feel it is unjust, you may request an initial review of the citation in person, by telephone or in writing. To initiate the process, complete the form and return it to the Community Improvement Department, 208 West Emporia Street, Ontario, CA 91762. The completed initial review request form must be received within 21 days of the citation's issue date to be considered. Please be specific in explaining why you feel dismissal of the citation is warranted.
